Rafa Leao closed the scoring with a goal in the second minute of overtime.
Milan will meet the winner of Atalanta and Sassuolo in the quarterfinals of the Coppa Italia.
The Rossoneri have five trophies to their credit, the last of which was won 21 years ago, in the 2002-2003 season.
