Seeded fifth in Bad Homburg, the Canadian won the match against Kartal 6-2, 3-6,7-5.
The first set belonged entirely to Bianca, who took a 4-0 lead, and it looked like the match would be over after two sets.
But Sonai Kartal made a great comeback in the second set and turned the duel into a deciding set.
Despite being 4-1 down, Andreescu lost again as the Brit won the set at 5-5 and the 2019 US Open winner narrowly avoided a tie-break and qualified.
In the 1/8 finals, Bianca Andreescu will meet the Spaniard Rebecca Masarova (69 WTA).
The Bad Homburg Open is the third edition of the WTA 250 grass court. The first two were won by Angelique Kerber (2021) and Caroline Garcia (2022).
