In just five minutes, the Argentinian Giovanni Simeone received two yellow cards and left Napoli with a numerical advantage.
The pressure from Milan followed, but after a goalless 90 minutes, it looked like the match was heading for extra time.
In the first minute of stoppage time, captain Lautaro Martinez scored from six yards after Pavard’s cross.
The team coached by Simone Inzaghi will win the eighth Italian Super Cup trophy.
