According to the document, the form has a special regime and is printed by specialized units on the basis of orders received from the National Agency for Fiscal Administration for the GUDF and regional GUs of finance for the tax audit service in them and for tax inspection structures for individuals in their subordinate units.
The main administration of personal income, regional main administrations of public finance and their subordinate units will take measures to implement the provisions.
